Job Description

St. George’s University Hospitals NHS Foundation Trust is the regional specialist care provider for south west London, Surrey and beyond. The trust provides a high quality, comprehensive range of health services from leading edge tertiary and trauma care to community services, delivered from 2 hospital sites, St.George’s Hospital in Tooting and Queen Mary’s Hospital in Roehampton, and from St John’s Therapy Centre, 11 health centres and clinics, schools, nursing homes and people’s own homes. The trust also manages the healthcare service at HMP Wandsworth. With national and regional referrals added to our local 3.5m catchment, we serve a population of around 5m. We employ around 8,000 staff.

We are looking for an enthusiastic and motivated band 7 OT with creative ideas on the future delivery of community rehabilitation to join our community adult therapy team. We are looking for them to bring comprehensive knowledge and experience of community services and an excellent understanding of health conditions, especially those associated with ageing. The aim of the service is to provide goal orientated rehabilitation to service users in their own home or in other community settings in order to enable people to achieve their full rehabilitation potential following injury or illness / to facilitate timely discharges and prevent avoidable acute admissions / to support people in the management of long term conditions / promote wellbeing and independence. The service incorporates both rapid, intensive input as well as longer term rehabilitation.

The successful applicant will be responsible for undertaking complex clinical assessments, identifying rehabilitation goals and performing triaging and case management roles. They will set and evaluate care plans including rehab goals, facilitate support workers in the delivery of the care plans, refer to other services, contribute towards Telecare assessments and line manage / develop other colleagues. They will drive forward excellence in clinical practice, promote evidence based practice and research opportunities. You must possess excellent interpersonal skills and have robust experience of working with a wide range of people especially the older population and in particular those with dementia. Experience of working in a rapid response team, the ability to use your own initiative and be an excellent communicator will be necessary. Candidates will be required to demonstrate the ability to supervise and develop other staff members and undergraduate students.

Access to your own transport (bike or car) is essential as our work is in the community. Please indicate how you would do this within your application.
The post holder will be expected to participate in a weekend rota on some Saturdays, Sundays and Bank Holidays.

There is a strong multi-professional team focus to patient care, we are a team of Occupational Therapists, Physiotherapists, Speech and Language Therapists, Technical Instructors and assistants. You will be supported by other experienced senior clinicians and a team leader. We provide rapid response input as well as longer term community rehabilitation. Continuing professional development is actively supported via clinical supervision, Personal Development Reviews, post-graduate and in-service training. Previous experience of working in the community would be desirable however, demonstration of strong organisational and time management skills would be equally important.

Community Adult Health Services (CAHS) is a care group within the Community Adult and Children’s Directorate of the Community Division and comprises multidisciplinary team delivery of functions of care related to a patient’s assessed needs. CAHS provides domiciliary rapid assessment of a change in health condition to support admission avoidance, supports safe and effective discharge and on-going treatment at home or in another setting following an episode of acute hospital care, provides urgent, scheduled and on-going input including specialist intervention for patients with long term conditions, provides in-patient and domiciliary rehabilitation to enable a patient to achieve and maintain optimum functional ability. CAHS is delivered from four localities; North Locality at Stormont Health Centre, South Locality at Tooting Health Centre, East Locality at Tudor Lodge Health Centre and West Locality at Westmoor Health Centre, Queen Mary’s Hospital Roehampton and St John’s Health Centre. The CAHS redesign is a new way of delivering care and is currently being implemented across the care group. We are exploring new and innovative ways of working utilising technology and mobile working to support staff in the delivery of high quality, safe and effective
