Following a strategic review, the Elderly Care team is redesigning the way it delivers services to frail older people. Developing a Rapid Elderly Assessment Care Team (REACT). The team aim to provide a multi-disciplinary, rapidly responding but comprehensive assessment service for frail older people.
The post holder will support admission avoidance and reduction in unnecessary prolonged hospital length of stay for frail older people. Working with the existing team of geriatricians and a Frailty Advanced Nurse Practitioner you will work to achieve the following:
- To avoid unnecessary admissions for frail older people by providing expert knowledge and support at entry points to urgent and acute care services
- To ensure that frail patients who are admitted have a comprehensive geriatric / therapy assessment and treatment plan to support timely discharge back to usual place of care
- To ensure co-ordinated care at discharge with Emergency Health Care Plans in place to manage ceilings of care on return to normal place of residence.
- To support the development of the REACT service and potentially deliver the service over extended hours.
- To provide a link between newly established community integrated teams, Haven Court, Social Care, General Practitioners and urgent care services for older people.
Applicants should have recent clinical experience working with frail older people.
The successful applicant will be able to demonstrate a track record working as part of the multi-disciplinary team, and have knowledge of the various community teams including but not limited to admission avoidance, integrated teams, intermediate care and End of Life which will allow them to instigate effective sign posting.
The individual must be clinically competent and have the knowledge, skills and experience to work as an autonomous practitioner in both the acute and community setting.
