Based in the Chard, Ilminster, Langport Integrated Rehabilitation Team. Fixed term contract to cover Maternity Leave We are forward thinking organisation looking for enthusiastic staff who are up for the challenge of working in a new and innovative way. The Integrated Rehab Team consists of Physio, OT and Rehab assistants working with community hospital and community setting providing rehabilitation to a broad range of patients within the South Somerset area. As a member of the team you will treat a variety of conditions, including the frail elderly, falls, orthopaedics and neurology. You will carry out specific Physiotherapy assessments, which will include goal setting, and treatment planning, assessing risk, promoting and enabling independent functioning, and ensure that service users optimise their rehabilitation potential. You will be committed to a person-centred approach to your work, have an ability to prioritise and work flexibly. We are a supportive rehabilitation service that is committed to continued professional development, peer support and practice education, and we have excellent support from our clinical specialists. This post will involve supervision of band 5 physiotherapists and band 3 rehabilitation assistants.
