Exciting opportunities have become available for Band 5 and Band 6 Physiotherapists to work within our multi-agency, integrated care service (ICS).

Working in a community setting offers the opportunity to gain a wide range of experience. You would be working within a team who are experts in delivering a rapid response and managing complex pathologies in order to prevent unnecessary admissions to hospital. Our teams also work closely with our partners in the acute setting to facilitate early discharge from hospital and provide rehabilitation services in a variety of settings to maximise patient independence.

The teams are co-located with Adult Care within Integrated Community Hubs meaning that you would be working closely with a multidisciplinary team to deliver a holistic approach to care. Not only is this great for our patients but it provides a real opportunity to learn from and be supported colleagues in order to develop your practice.

The successful candidates must be highly motivated with excellent communication skills, have the ability to work on their own initiative, but also be an effective team worker. The post holders will be expected to work flexibly, and this will include working weekends/bank holidays on a rotational basis. They will be expected to travel regularly throughout the locality of the team and therefore access to transport is essential, pool cars may be available in some locations.

You will need to effectively engage with your patients, peer groups, and service users in all elements of your role, leading by example and promoting a supportive and caring culture. This will require an inclusive, transparent, open approach to your role, whereby you are happy to be accountable to others and to go that extra mile when required. You will be passionate about our business, creating a culture of continuous improvement, where patient welfare is at the heart of all you do.
