Wiltshire Health and Care is formed from a partnership of the three NHS Foundation Trusts who serve Wiltshire, Great Western Hospitals NHS Foundation Trust, Royal United Hospitals Bath NHS Foundation Trust and Salisbury NHS Foundation Trust. From 1st July 2016 Wiltshire Health and Care will provide adult community health services in Wiltshire. As part of the team you will help deliver services to the Wiltshire Health and Care contract, however your employment will be with one of our partner organisation, Great Western Hospitals Foundation NHS Trust.
Trowbridge Community Team provides nursing, therapy and support workers for the people of Trowbridge and the surrounding area. There is an opportunity for a secondment of initially 6 months into the community as a Band 6 Physiotherapist within our Community Team, based at Trowbridge Hospital, with a possibility of this being extended. The post is for 30 hours per week Our physiotherapists are expected to work seamlessly within the multi-professional Integrated Community Team and to forge close links with other local teams. We are currently working in a Rapid Response Model of working and in the future hope to look at running balance and falls clinics with Primary Care. Members of the Intermediate Care Team, Including the Clinical Lead for the West a New Post within Intermediate Care are based with the Integrated Community Team. There will be opportunity for rotation and development; the post will be varied and interesting for any Physiotherapist. Significant experience as a Band 5 physiotherapist in front line NHS community patient care is essential as is evidence of leadership and management skills in practice. We would like to see clear evidence of up to date and relevant clinical practice and education. Applicants need to have excellent clinical skills, well developed communication skills and be able to work under pressure. The team operates a shift pattern from 9-5 and 10-6 at the present time. Therapy staff will be expected to work weekend on a rota basis. All new starters with Wiltshire Health & Care will be subject to a six month probationary period. The probationary period works alongside the induction process to help create a positive and supportive working environment that allows new staff to learn the key elements of the role within a reasonable and realistic timescale.
