The South Manchester GP Federation (SMGPF) has been commissioned to provide a team comprising six GP sessions, eight nurse sessions and 14 health care assistant sessions per week. This is a pilot for a year and the team has been commissioned to support practices (up to thirteen) in South Manchester to improve outcomes for patients with long term conditions (linked to local standards).
The team will be operating in the practice locations and will be delivering one of the following options in the practices:
The HCA would be accountable to the Federation’s Project Manager but reporting to the practice manager for the practice the HCA is working in. The HCA will be required work on a sessional basis and required to work at all times within the job description details stated and within their capabilities.
The ideal candidate should have a minimum of 2 years HCA experience and experience working in a GP practice.
This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
