With significant new funding from the Vanguards programme in South Somerset we’re redesigning the way primary, community and acute care services work together.
With a focus on providing our patients with access to the right care, in the right place, and at the right time, we are creating new roles which will underpin this exciting collaborative approach.
Our Symphony Complex Care Hubs are providing an assessment and care coordination function initially 0830 to 1800, 5 days per week moving towards a 7 day service. We are currently looking to appoint a Professional Lead to manage and support the complex care hub. Our Hubs provide an assessment and care coordination function and the staff working within the team work on a rota covering shifts from 0800 to 1800 (7 days per week).
The Symphony Complex Care service includes:
• A comprehensive assessment of the individual’s physical, mental health and social care needs resulting in the development of a single personalised care plan with patient centred goals and measurable outcomes. This care plan will be available to both primary and secondary care and other partners in a timely way;
• Support and coaching for patients and carers to understand and manage their own conditions, and to link with other sources of advice and support;
• Routine contact and monitoring of patients’ and carers’ health conditions and care needs, incorporating effective use of emerging new technologies;
• Escalation plans ensuring that patients and carers recognise deterioration triggers, and that there is a co-ordinated and planned response.
This complex care model builds on the existing FOPAS service, which already provides significant elements of the rapid response service.
The post holder will have responsibility for operationally managing the Nursing, Key worker and Administrative teams within the hub. They will be responsible for providing the strong leadership and management skills required in order to direct the Team in the provision of the highest quality care for patients with complex needs. The post holder will have excellent team building skills and previous experience in implementing new services or delivering change within a small team.
