Service Care Solutions are currently recruiting for an Adults Social Worker to work within the Hospital Team based in Liverpool.

This is a fantastic opportunity to work for a well-known local authority and the role will include arranging safe and timely discharges for service users, commissioning packages of care, managing a varied caseload, attending continuing healthcare meetings and completing Capacity Assessments.

In order to be considered for this role you will need the following -
•Social Work degree or equivalent
•HCPC Registered
•Fully enhanced DBS
•Post qualifying experience in a social work role
