Job Description

We are looking to recruit a band 5 Nurse who has an interest in community working to join our established team of experienced clinicians. The individual would be responsible for a delegated caseload having senior support at all times to manage a diverse caseload of clients in the community.

This is a unique role that will ensure that the post holder will develop in their holistic assessment skills and knowledge. Specific areas of assessment will include Falls & Fragility Fraature Risks as well as Continence needs leading onto full care planning and advice being provided to the client. Clients may be seen in a variety of settings inclusive of clinics, residential/nursing homes and community.

The individual will be clinically supported by senior clinicians in the three specific areas, there will be opportunities to shadow practice and have regular supervision to discuss cases. This is a fantastic opportunity for anyone wishing to develop their portfolio and have assurance that support will be available.

The successful candidate must have good communication skills, the ability to work as part of a team, have an understanding of confidentiality and ability to prioritise their caseload and be and effective manager of their time. As this is a community post, the ability to commute within the Trusts geographical area effectively is essential.
