The Core function of this post is give rehabilitation support to the Service User’s, to follow their treatment plans, strictly adhering to protocols and policies in line with CQC requirement without supervision whilst reflecting individual choice and client’s independence. It includes identification, delivery and evaluation of professional rehabilitation interventions to mentally/ physically/ sensory impaired people to enhance their skills and confidence to maximise their independence by completing assessment of need, care coordination, agreeing and implementing care and support plans and the reviewing function. Mentors and counsels Service User’s, families and carers with mental health problems.
Rehabilitation Officer facilitate Home Support/Hospital Discharge Service programme, which is ideal for clients who require support and assistance with the activities of daily living. Rehabilitation ensures that the needs of people using the service are fully taken into account at all times, including during unsociable hours and at weekends. Work as a part of Multidisciplinary Team.
You will possess excellent verbal and written communication skills, a firm grasp of Safeguarding policies, have a sound working knowledge of this service user group which includes adults with sensory needs. You will have a good track record of working with the Independent mentally ill residents.
