Purpose of the position
An exciting opportunity for a Nurse Specialist has arisen in North Lincolnshire. The key purpose of this role is to deliver a commissioned community respiratory service within the North Lincolnshire CCG . They will work with the Team Leader and Respiratory Physiotherapists with specialist input from the MDT to ensure improved management and outcomes for people with respiratory disease in North Lincolnshire. The programme provides the on-going delivery of support and management of respiratory patients within North Lincolnshire. Home assessment, exercise and education to patients within a group setting, providing self management support. The role holder will be required to manage their own caseload, promote the service locally to increase appropriate referrals.
The position reports to the Team Leader. Respiratory Nurse Specialists will be aligned to one part of the service, (pulmonary rehabilitation, home oxygen assessment & review, self-management, respiratory care) however all post holders should be flexible to work across all parts of the service to meet patient volume demand and provide cover as required
