Solent NHS Trust is looking for compassionate and dedicated nurses to join our Community Health teams to provide exceptional health care whilst developing your skills and expertise with us. This is an exciting time and in recognition of the important role Community Nurses play in supporting patients at home and in our Community Hospitals, investment has been made in a number of new nursing posts. We are keen for you to join our forward thinking teams and we are keen to support you in your Preceptorship/Mentorship in a variety of healthcare settings.
We have a variety of positions at band 5 level across all services with opportunities to work a variety of hours across the 24 hour period. We require both shift workers but also staff to work Monday to Friday 830 – 5pm but we are very flexible with hours worked.
We are a large and diverse service that provides Community services mainly in Southampton City but also small teams in Portsmouth – our services are provided not only in patient’s homes but also in Community Based Hospitals and also in a variety of clinics. Community Nursing including Case Management and Palliative Care:
Community Nursing are Locality based services that provide nursing support to adults and older people who are unable to attend their local GP surgery, or where is inappropriate for them to do so. There are 3 localities in Southampton in which the teams work closely with GP practices, the key objectives are to:
The aim of Rapid Response is to avoid hospital admissions where possible and assist with hospital discharges from the acute trust, offering a 235 hour service.
Community Emergency Department Team: this team is based in the Emergency Department at UHS with the aim of the service to promote the care of patients who are 65 + in their own homes rather than in the acute hospital by providing specialist knowledge on services available in the community.
Long Term Conditions, Cardiac, COPD, Diabetes and Bladder and Bowel:
Bladder and Bowel service promotes continence and management of continence through interventions by specifically trained nurses, screening for prostate cancer and specialist advice and treatment for urinary tract problems is also provided.
Community Rehabilitation at the Royal South Hants:
Two wards with 43 beds at the RSH provide consultant led elderly care rehabilitation for patients.
Snowdon Ward – Consultant led unit with 14 specialist Community Neurological Rehabilitation inpatient service. Provides intensive follow on rehabilitation at home to support transition into patients own environment
Kite Unit (Portsmouth) – Consultant led 10 bed specialist Neuropsychiatric Rehabilitation for patients with brain injury whose impairments and largely behavioural, cognitive or mental health related
