The Digital Care Hub provides video and telephone support to over 10 000 patients living in their own homes and in nursing and residential care.
The Hub is a 24/7 service, with the aim of supporting patients in their own place of residence where it is clinically safe to do so, reducing the number of unnecessary attendances at A&E and unplanned admissions.
The hub provides a range of services alongside supporting over 400 care homes, Including Gold Line, Intermediate Care, Complex care and shortly Craven District nursing team
The service continues to expand and as such we need additional staff to offer greater support the new innovations in our work. Our Vanguard work is getting well established and we are looking to roll out this work further.
