Are you looking for a new challenge? Are you an experienced Community Nurse looking to work OOH supporting patients in their own homes? The Integrated Community Nursing team has an exciting opportunity for a new role within the Night Service to support this vision.
At UHSM we have an established Community Nursing Night Team which operates between the hours of 2145 and 0815, providing high quality assessment and care for those patients who require both urgent assessment of immediate needs and ongoing nursing care in their home environment.
Staff members work in pairs throughout the night shift to facilitate safe working practice. The work is varied and interesting, and will require knowledge and experience in the management of palliative care patients, as well as competent assessment and intervention of immediate needs to prevent unnecessary hospital admissions.
The role requires a dynamic, enthusiastic and motivated individual with effective communication skills and the skills to coordinate care, for patients in the OOH period. You will act as an advocate for patients residing in the community, and have the ability to inspire people at all levels. It will be expected that you will have an understanding and commitment to the modernisation of community nursing.
The shift patterns will be allocated on a flexible rota pattern, with each shift lasting 95 hours and based at Withington Community Hospital.
Travel is an essential part of this role; please demonstrate on your application form how you would meet this requirement.
