Dundee Health& Social Care Partnership based at Maryfield Medical Centre, Dundee Social Prescribing Link Workers – Indicative Band 6* Post A: Practice Based Link Worker Post B: Locality Based Link Worker Fixed Term/(*Secondment for internal applicants) from December 2016 to March 2018 Salary Scale : £26,565 - £35,577 per annum (*Salary and band are indicative only and are subject to Agenda for Change job evaluation) Over the past five years, Equally Well in Dundee has been a partnership between the Scottish Government, NHS Tayside/ Dundee Health and Social Care Partnership, Dundee City Council and the community and voluntary sector. Opportunities have arisen for experienced applicants to act as link workers in an established Practice based setting and to test a similar approach within one Health and Social Care Partnership locality. Post A link worker will provide a service to referred patients from identified GP Practices to support them to improve their physical and mental health and wellbeing, and reduce frequent attendance in primary care, by accessing a wide range of non-clinical services and activities. The link worker will help recruit, train and support volunteers to provide supported access to referred clients. Post B link worker will establish social prescribing in other settings beyond primary care within one designated locality and will support frontline staff from different services to adopt social prescribing approaches as part of their daily business. We are looking for autonomous practitioners with experience of community based, person-centred, non-clinical approaches; collaborative, multi-disciplinary working; and skills in training and capacity building. Applicants must be educated to degree level in a relevant area, preferably with evidence of post-graduate training or qualifications. It is desirable that applicants have previous experience of working within social prescribing schemes.
