Job Description

Wiltshire Health and Care is formed from a partnership of the three NHS Foundation Trusts who serve Wiltshire, Great Western Hospitals NHS Foundation Trust, Royal United Hospitals Bath NHS Foundation Trust and Salisbury NHS Foundation Trust. From 1st July 2016 Wiltshire Health and Care will provide adult community health services in Wiltshire. As part of the team you will help deliver services to the Wiltshire Health and Care contract, however your employment will be with one of our partner organisation, Great Western Hospitals Foundation NHS Trust.

An exciting Fixed Term Contract opportunity has arisen to work in the Chippenham Community Team. Our Community Physiotherapists are expected to work within the multi-professional team to provide a seamless service and to forge close links with other local teams. The team is based within Chippenham Community Hospital and operates a shift pattern from 0700 – 2200. This role aims to deliver an integrated care model that supports a range of District Nursing and therapy services. The post holder will require excellent communication skills and will be working within a multi-disciplinary team providing a timely, flexible response to referrals, supporting people in their homes, providing specialist clinical input and treatment as well as in reaching to rehabilitation beds in a local residential/nursing home. Some clinical experience in the community is desirable; as is evidence of team working and the ability to work flexibly within a busy demanding service. The successful candidate will support the multi-disciplinary team and work closely with GPs and Acute hospitals. We provide clinical supervision and therapy training within the team and the locality. The Academy also support further CPD for courses identified within appraisal. Due to the nature of the role it is essential that applicants are able to travel independently across the locality and have access to a suitable vehicle for business purposes.
