The Rehabilitation and Therapy Service part of Guys and St. Thomas NHS Foundation Trust, Adult Community Services is looking to recruit a highly motivated and enthusiastic Band 6 Community Physiotherapist. The Pathway covers both the Boroughs of Lambeth and Southwark.This is an excellent opportunity for a Band 6 physiotherapist or an experienced Band 5 Physiotherapist.
Applicants should have experience of delivering high quality; evidence based rehabilitation to older persons with complex needs, multiple co-morbidities and falls. The post holder will be required to support service developments and show flexibility when adapting to and leading change.
The post holder will work alongside a multi-disciplinary team to implement timely and person-centred assessments and rehabilitation. The role includes partnership working across teams within the Rehab and Therapy Services and other agencies within primary and secondary health and social care.
Excellent communication skills and an ability to prioritise a large caseload of complex patients are essential.
The post holder will be required to contribute to the 7 day therapy service on the Pulross Intermediate Care Ward.
