An exciting opportunity has arisen at St Luke’s Hospital, for a Band 6, Senior Physiotherapist to join our well established virtual ward team, working in the community across Bradford. An interest and experience in general rehabilitation and discharge planning is essential.
As a Band 6 Therapist you will be part of a multidisciplinary team comprising of Geriatricians, Advanced practitioners, Occupational therapists, nurses and rehabilitation support workers. The Virtual ward is integrated with our social care and voluntary sector service colleagues.
The Virtual ward team is innovative, creative and friendly. This well-staffed team works generically, providing high quality care through effective team work. We are committed to continuous personal development and you will be actively involved in teaching, CPD, In-service training and MDT working. There is ample opportunity to develop clinical supervisory skills as we regularly take students.

Summary of most important Experience/Skills/Qualifications required:
The post holder will work across the integrated virtual ward team (Acute, social and Primary care)
Drawing on their clinical expertise in order to undertake advanced assessment, enablement and
discharge planning. It is essential the candidate has a good background in older people experience
with older people and some experience with declining cognition (delirium/dementia)
It is essential that you have a full UK driving licence and have a car available for work
