Avon and Wiltshire Mental Health Partnership NHS Trust (AWP) is a significant provider of high quality mental health services across a core catchment area covering Bath and North East Somerset (B&NES), Bristol, North Somerset, South Gloucestershire, Swindon and Wiltshire. The Trust also provides specialist services for a wider catchment extending throughout the south west.

Specifically, The Trust provides services for people with mental health needs, with needs relating to drug or alcohol dependency and mental health services for people with learning disabilities. We also provide secure mental health services and work with the criminal justice system.
Increasingly AWP provides treatment and care in people's own homes and other community settings, reflecting the preferences of our service users. Our community services are supported by high quality inpatient services that provide short term assessment, treatment and care.
The South Wiltshire Intensive Team is looking to recruit a specialist practitioner to join a very experienced and supportive team. We work with people who have short-term and longer-term mental health problems who are currently are in crisis. The team provides emergency assessments for people and provide short-term interventions (4-6 weeks) focusing on individual recovery and resolving the crisis.
The team has a full-time team manager, dedicated full-time consultant psychiatrist and a multi-disciplinary team consisting of specialist practitioners, practitioners and recovery workers. We also have access to psychology input to support assessments, formulation and care planning. The team also has a dedicated admin resource.
The team supports individuals to remain in the community in the least restrictive environment where this is the best option and also facilitates early discharge from hospital. Supporting carers is a frequent part of the role.
The Band 6 role predominantly involves working with service users to complete the initial assessments, devising and delivering the follow up care plans with the support of the MDT. In addition the role contributes to the overall management of the caseload and supporting the team supervision structure.
The Team operates over a 24 hour period including waking nights.
