Our clients are a group of 12 individuals who have many and varying COMPLEX NEEDS - The things we as humans take for granted are things that our clients cannot do for themselves. Feeding, walking, Speaking, moving are just a few.

We are seeking an RGN, RMN or RNLD who is passionate about making a difference to the lives of these individuals.

Job Summary

To provide a lead role in supporting a group of individuals with learning Disabilities with complex needs to live & network in the community providing Health & Social support in a Personal Centred approach . Be responsible in ensuring that individuals have care plans which are reviewed Regular and are in the best interest of the service user. Work as part of a team, assist the manager in planning & Development service in order to meet the CQC compliances. Maintain approach to support which encompass the rights & dignity of each individual in order to achieve ambitions & acknowledge the Diversty,integrity ,performance, challenging Barriers & Pioneering Vibrancies values into action ) .

Principal Duties and Responsibilities


  1. To make a thorough assessment of the person's needs in a personal centred approach in order to meet ambitions & achievements
  1. To assist in ensuring all residents or their relative/advocate and staff are aware of, and understand the Complaints Procedure and that it is rigorously followed. in appropriate Language
  1. Top ensure that Vibrancies Diversity Strategy is implemented. By Valuing each individual differences by being learning & challenging


Ensure the maintenance of good relationships with individuals, team members, relatives and the wider community.


To ensure that the Operational Policy is adhered to in all respects. Being fully aware of vibrancies Objectives

To undertake work as reasonably requested by the Project Manager.& support the team ensuring CQC compliances are adhered to

To assist the Project Manager in planning, implementing and monitoring Individual Programme Plans.

To be prepared to attend review meetings, courses and training centres when required and to contribute to the assessment of the strengths and needs of individuals on personal centred plan basis and ensure person centred approach to health and social needs. Liaoning with Health care professionals ,relatives to ensure the best interest of the individuals are met

To be familiar with the steps to be undertaken when medical emergencies arise. In ensuring their continuation of the individuals are maintained in a personal way

Challenging Barriers

To participate in all activities within and outside the home, including cooking, shopping, cleaning etc. as needed. To support service users Holidays, Medical Appointments. Being flexible and responsive to the wishes of the service users contracts

To attend meetings and provide written report as required. and to have the necessary it skills to better individuals around their needs in sharing information

To be aware of personal responsibilities regarding health and safety at work and fire procedure.


To demonstrate relevant professional skills to other staff members enabling them to undertake their appropriate duties & to promote working together to achieve common goals

To provide a lead role in key areas of responsibility within the house, supervision,training supporting needed for the job To supervise the work of subordinate staff and introduce new members to their duties.

  • Participate and contribute to one's own appraisal and supervision session ensuring values and standards are maintained.
  • To ensure National Minimum Standards are adhered and in line with Care Quality Commission Regulations and are able to make sure that their standards are implemented and maintained in line with regulations, and to achieve maximum rating in providing good quality of service.


  1. Provide cover for colleagues as directed by your Manager.
  1. Refrain from acting in a manner which in any way endangers yourself, fellow employees or the public.

  1. Safeguard at all times confidentiality of information relating to staff and clients.
  1. Refrain from smoking in any area of Vibrancies premises not designated a smoking area.
  1. Behave in a manner which ensures the security of Vibrancies property and resources.
  1. Abide by all relevant Vibrancies Policies and Procedures.
  1. Be prepared, if required, to participate in a clinical on-call rota.
