The service is a residential site providing person centred support to people within a diagnosis of Learning Disability, Autism, Physical Disabilities and some Challenging behaviours. The service provides 24 hour support and the level of care offered depends on the individual needs. All individuals are encouraged to be involved in designing their own person centred support plans with the staff team. We encourage and support those at the home to recognise and identify their goals, ambitions and to be as independent as possible.
We require Support workers who can provide support and encouragement to staff and residents in all areas/activities of daily living. Working shifts during Monday to Sunday 52 weeks a year in a full time post. This includes sleeping nights, nights, working weekends and Bank Holidays. Duties include contributing to the assessment of residents needs and the development of individual care and support plans, in consultation with service users, their families and staff. Conducting risk assessments where required, Prepare incident reports and update daily records and logs as required including adhering to policies and procedures. Maintenance of safety, security and confidentiality. The candidate must have the ability to lead a team confidently.
