Team Leader – Education and Health Care Plan Team
Band 7 – Registered Nurse
Central London Community Health/ London Borough of Merton
375 hours per week ( whole of year)
We are looking for an experienced, passionate Registered Nurse with paediatric experience to lead our multi-disciplinary Educational Health Care Plan Team in Merton Borough. This is a unique and developing role to support health delivering within the EHCP process working across health, education and social care.
Your role will be to coordinate the health advice for, children and young people (3-19yrs) going through the educational health care plan assessment process and to support and team managing Occupational therapy, CAMHS work within the EHCP team. The role will include carrying out and coordination health assessments (where required) and is based at Merton Civic Centre but covers work in the community eg: clinics/ schools and families homes. This is a unique opportunity for the right person to implement, develop and build health input for Educational Health Care Plan services across Merton whilst liaising with our external partners to support hospital discharge, health assessments in Merton, identification of Looked after children’s health needs and supporting social workers, education staff and health staff meet the needs of children and young people in Merton.
You will work closely with established multi-professional teams within Merton Community services and with outside agencies to support identification and delivery of health needs and Educational Health Care Plan support eg: attending joint appointments with external partners, identifying, assessing and coordinating health needs of children and young people, requesting funding for health needs and working with acute hospitals to meet the needs of Merton families. You will be able to carry out formal and informal health assessments in children and young people with identified complex health needs at home, school and in respite. You will have had significant experience of working with a variety of complex needs in the acute/ community setting/ home settings, have a passion for developing a multi-disciplinary team and outstanding communication/ team working skills across different agencies.
We are committed to professional development and evidence based practice, offering regular in-service training and peer group / one to one supervision within the wider CLCH family. Internal and external courses are supported. You will be required to have at least 2 years’ experience working within and field of children’s nursing, appropriate transferable skills working with children and young people with complex health needs. You should have experience of engaging in appraisal processes, working within a multidisciplinary team and be willing to further develop your management and leadership skills.
