Team Manager – To work in the Referral Centre
Salary: £31,072 - £40,964
Hours: 375 per week
Location: Forest of Dean, ICT
Gloucestershire Care Services deliver community health & social care services for adults & older people via a model of locality based Integrated Community Teams (ICTs).
The ICTs consist of community nursing staff, therapists and reablement workers, with social work professionals also working alongside us. The teams work closely with a number of partners including GPs (encouraging a case management approach), the community hospitals and other partners, to enable people to live as independently as possible within their own homes. The ethos of the teams is to provide a reablement and person-centered approach to recovery and self-management, ensuring that we offer care and support close to home, thus avoiding unnecessary admissions to bed based services.
The post provides an excellent opportunity for someone to join the Forest of Dean locality to support the ongoing development of the Referral Centre and lead an integrated multi-disciplinary team to deliver a referral and response service to the population of the Forest of Dean

You will need:
