We are looking for an enthusiastic Community Therapy Assistant who would like a Band 3 post within a friendly multi -disciplinary team. Would you enjoy the challenge of a varied caseload in a variety of settings providing rehabilitation for Older Adults and a Rapid Response service to prevent unnecessary hospital admissions for the population of Wokingham? As a valued member of the team you will have the opportunity to develop rehabilitation and basic nursing skills in a proactive and supportive environment.

  • You will be working with patients who require rehabilitation and/or palliative care. You will be working together with Therapists and Nurses to help prevent unnecessary hospital admissions.
  • You will have the opportunity to complete the Care Certificate.
  • You will help to develop this exciting service to meet the changing needs of our population.
  • The Intermediate Care Service is a 7 day service and operates from 0800 – 2200hrs. You would be expected to be able to cover all aspects of the shift pattern.

The successful candidate good time management skills and be computer literate. You will have the chance to be part of an innovative integrated Health and Social Team. You will be based at the Old Forge, Wokingham.

The successful applicant will be joining the team during a very exciting period of change with the integration of Social Service Hospital Liaison team and the Intermediate Care Team. The team provides End of Life care, Rapid Response Hospital Avoidance and Reablement to the residence of Wokingham.

The primary role of the Intermediate Care Sister is the identification and delivery of health, social and care needs to those patient’s admitted to the service. The service aims to prevent patients having inappropriate hospital admissions and expedites hospital discharge when appropriate.

The role encompasses leadership and judgement in the management of junior members of the team to ensure good clinical practices and high standards of care are maintained in order to meet service demand.

To assessing and developing students and junior staff to reach their full potential by designing learning programmes and acting as mentor.

The post requires 7 day working from 0800 – 2200. The applicant must have a clean driving license and have access to a car.
