Weldmar Hospicecare is looking to appoint a Weldmar Community Nurse to deliver integrated and holistic palliative care services to patients, families and carers in the Central Dorset area.
Working within the Integrated Team the postholder will respond to patient needs as determined on a daily basis under the direction and guidance of the Weldmar Community Nurse Lead/Assistant Director. They will provide the highest standards of clinical care to the patient and family at home, hospital, clinics or social day respite services, through excellent assessment, symptom management, emotional support and access to appropriate services and advice. They will practice evidence based palliative and end of life care, holding a flexible caseload responsibility.
The role of Weldmar Community Nurse includes developing effective networks and partnerships that support the provision of guidance, education and mentorship to partner professionals across the locality’s health and social care community e.g. GP’s, DN’s.
The postholder will be supported into this new post by the Clinical Leadership provided by the Weldmar Community Nurse Lead and other professional members of the Central Integrated CommunityTeam.
