- References covering the three year period prior to registration
- Professional registrations (where applicable)
- Additional declarations including, Caring for Children, Disclosure of Criminal Convictions, rehabilitation of Offenders Act and Data Protection Act.
- Enhanced Criminal Record Bureau/Disclosure Scotland disclosures
- Proof of identification
- Proof of Eligibility to work in the UK
