The Royal College of Nursing surveyed its members about the use of social enterprise to provide health care.
It also asked about changes that might include third party providers, taking over some of the functions of hospitals or running primary care.
The survey found nurses agreeing to change in principle - but highly concerned about the detail.
As many as 100 per cent of RCN members said they backed change.
But this fell to 57.5 per cent when asked if they would transfer their employment to a social enterprise - and even then only on the condition that the nurses could retain their NHS terms and conditions of employment.
Some 96.5 per cent agreed with the principle of flexible ways of working.
A spokesman said: "The key focus of the RCN survey was to identify find out how much its members knew about social enterprise models and to what extent they supported or were concerned about opening up the market to third-party providers."
