A number of exciting opportunities to work within the community integrated teams across Central Manchester have arisen. You will be responsible for delivering nursing care to patients within their own homes and clinic settings. You will prioritise, assess, plan, evaluate and deliver all relevant aspects of nursing care to an agreed and expected high standard. Working as a key member of the multi-disciplinary team you will be able to make a difference and contribute to the processes that promote patient safety and quality of care in line with clinical governance frameworks and Trust Values.

The Division of Medicine and Community have a vision to deliver integrated care across health and social care boundaries within Manchester. To achieve this vision we intend to educate and develop a flexible, multi-skilled nursing workforce able to challenge and change practice, reduce service barriers and provide care where it is needed within an established team. You will receive a comprehensive induction, regular 1:1's and annual appraisals to develop knowledge and skills where appropriate.
